ILN Questionnaire

The Integrated Living Network (ILN) was set up in November 2006 with the aim of supporting the housing sector to improve the health and well-being of the tenants and the communities in which they live by working in more effective partnerships with health and social care.

We are carrying out a review of the Integrated Living Network, with the aim of focussing future services more closing around the key challenges which you face.

I'd be grateful if you could spare a couple of minutes to complete this survey. The results will be used to inform the Integrated Living Network work plan for the next 2 years.
